Charges and coverage for 99455

This is a patient that is new to us and he states that he has been on disability since 2003 and has to have the paperwork done annually to keep the disability. He has a Medicare Advantage plan through AARP. I am in AZ.
I need to know how this is billed out and will it be covered? This code does not show in the Medicare 2014 fee schedule. Is there another code that should be used? This was billed by a new NP.
