Documentation for billing 99211 and a ua

There is a little confusion as to what needs to be documented to bill a 99211 and a ua, I know there needs to be proof of face to face time by nurse or medical assistant documenting complaint and discussing with md to come up with treatment or plan.. My qx is does the patient have to wait in the office and be notified before they leave to charge the 99211.
