physician co-signature

Does anyone know of a guideline that says a physician or other health care provider must rewrite a note in his own words for a patient that was also originally seen by an intern? Is the co-signature only by the physcian or other health care provider enough? By co-signing the note, wouldn't the health care provider be assuming responsibility for the note without having to re-write it?

No, a co-signature is not enough for GC modifier!! The teaching physician is for physicians and residents. Non physicians cannot supervise residents. Look on web for teaching physician rules, Medicare.

MY understanding is that a co-signature is not enough for me to bill for seeing the pt. Even if you don't charge for the pt, you still need to have some kind of documentation that you actually "saw and examined" the pt. My hospital requires that each pt be personally examined by a "medical staff member", so this applies to advance practice people's notes also.
