PT Modifier

With new PT modifier for Medicare, what is primary diagnosos code 45385 PT with V76.51 or 211.3 as primary.

PT modifier!?!

Our clinic uses the V76.51 dx code for the 45385 colonoscopy, but our claims have been rejecting through the contract carrier because the -PT modifier was "Inconsistent with the procedure." I contacted Cahaba and was told the changes Medicare implemented on 1/3/11 have been causing claims to deny in error so written appeals will have to be mailed to Cahaba. I have been removing the -PT modifier because the deductible is still being applied to the screening-turned-diagnostic procedure codes.

The instructions say a new HCPCS modifier so I am refiling with the screening code and putting V76.51 as primary code and 211.3 as secondary.
