pain management codes

How would you bill a lumbar epidural injection with bilateral one level facet block with Fluoro. I billed 64493 with 50 mod. with 62311 with 59 mod. and 77003 for the fluoro with 26 mod. Only got paid for the 62311 code. Denied everything else.
