Rfa & bone biopsy

Hey Guys,
Sorry if this is a dupe, but i had a password issue.
Can we bill for an RFA and a bone biopsy at same session?....not bundled, and carrier is denying the RFA and paid the bx.
Can someone help me with this?

Usually only RFA if definitive diagnosis. Use 59 if separate and distinct biopsy

So, if the biopsy is being done of the same lesion that is being rfa'd, we don't bill the biopsy? If it were a separate lesion from the one having the procedure, then we would bill for it...otherwise, not?
