Ophthalmology ICD 9 for E/M as well as F/U

Hello,

I have been receiving denials from Medicare when I bill 92004 with 92015 saying it as a routine visit as well as also for follow up visits when I bill it with 92014 or 92012 along with 92015.

Can anyone give me a link or list from where I can find out appropriate ICD 9 for ophthalmology that are not considered as routine for CPT® 92004 as well as 92014, and 92012.
