Office visit which led to EGD w/ peg placement, same day

I would like to seek your opinion on a coding issue that hasn't come up for me before. Pt was seen in office for anorexia, wt loss, vomiting, abl pn. It was determined at that office visit that an outpatient EGD needed to be scheduled that day.

Since the decision for surgery was made at the office visit, can I code the E&M with a 25 modifier and code the procedure 43246 also?

MOD 57 is more appropiate

Since 43246 has 0 global days a 57 modifier would not be appropriate.
