Facet Blocks and Lumbar Epidural Steroid injection

Hi Everyone,

I work for a Pain management doctor and I was wondering if anyone bills for facet blocks.

Scenerio: Patient presents with lumbar radiculopathy and Facet arthritis.

Doctor does lumbar epidural injection at L2-3 under fluoroscopy.

Then doctor does 2 level paravertebral facet block at L2-3 and L3-4 on left side.

Can you bill 64490, 64491, and 62311 all at one setting. Do I need modifiers?

I would look at 64493 and 64494. I would code first the 62311, then 64493 mod 59, then 64494.
