Excision and intermediate closure code for benign skin lesion.

If bx report of benign cyst size is 2.8x1.9x1.6 cm of chest wall .

Will be excision code 11403 and closure 12032 ?

Is this right ?

Do you go by the biggest size or you multiply 2.8x1.9x1.6 ?

The doctor should really be dictating the size of the lesion and margins that he took in his report, but seeing that you only give us the measurements from the path report, you have to go off of that. (The specimen will shrink so reimbursement will be lower if only going off the path report). The codes are correct but you don't need to add -51 modifier as CCI edits allows both codes to be billed together without modifiers.

Look at the editorial comments in the CPT® book that describe the excision codes to see the instructions about measurement. Then, look at the editorial comments in the CPT® book about repair to see when you can code the repair as well as the excision.
