proper coding for CPT® 57500 and 99395

How do you code well woman exam 99395 (what pt initially came in for)
found endocervical polyp and was removed and sent to pathology benign.

I used

99395
57500-25
99213-25

Which I know I should not have put 99213-25 but Insurance states 99395 cannot be used same day as procedure. Now we were not paid enough for removal of polyp. How do I appeal again?
