Chief Complaint

Does anyone know if there are any DG that state the CC needs to be documented specifically in the History portion and or in a statement by itself? Or is it allowable to extract it from the Plan/Assessment most specifically for a subsequent hospital visit?

Per the 1995 DG, "The CC, ROS and PFSH may be listed as separate elements of history, or they may be included in the description of the history of the present illness."
