WC denial

I bill for EMG-NCV and intraoperative monitoring. Liberty Mutual keeps denying SSEP (95925) of the upper extremities when monitoring low back surgery. We have sent in appeals stating this is the standard of care, documentation from CCI edits, documentation from AANEM, and have asked them to send us the guidelines they're using to deny-all to no avail. They still deny stating it's not medically necessary. The doctor has even spoken to the ins co, but still denied. Any suggestions??
