Anesthesia Modifiers

I read one of your articles about Anesthesia modifiers, but remain a little confused about the use of more than one modifier at a time. Can an anesthesiologist report an anesthesia service and append the QK and the GC modifier on a single line? What about reporting the AA and the GC together on a single line? I'm wondering if the anesthesiologist or the billing company typically submits claims for both services on a single line or if this is simply a billing error. Your help in clarifying is appreciated.
