Need to file an additional unit of injection missed on 1st claim submitted

Original claim was filed with only one unit of injection billed. Performed audit and realized additional 3 units need to be filed. Sent NEW claim for 3 units to insurance. Insurance denied as duplicate. Any ideas how to submit for missed units?

You will need to file as a corrected claim with the correct number of units.

No need to resubmit with modifier? I just need to attach corrected claim? Thank you.

Since they already processed the claim, you may need to send in a reconsideration explaining the clerical error. If it's Medicare, you can do that over the phone. A lot of carriers have their own form now so it's really easy. I'd start off with just calling them and asking so you won't have to redo it again, they may give you a fax number as a short cut.
