skin lesions

What CPT codes would I use for a patient that had 3 lesions removed including one vulvar, one perineal and one perirectal?

Need to know what type of lesion, skin (benign or malignant). What technique and diagnosis codes.

Benign lesions removed with a scalpel. ICD 239.5.

Excisions of skin lesions(through the dermis), is coded by Benign or malignant. Benign is code range 11400-11446). The excised diameter by cm must be documented, including margins. See body sites for accurate coding.

Do I code a single CPT or separate codes for each lesion?

If each lesion required a separate excision, then each lesion excised is coded. If the same CPT code, put modifier -59 on second lesion, same code.
