CCM 99490 for hospital based physican practices
We have 15 offsite physician practices that are part of the hospital... Can these hospital based providers (POS 22 outpt hosp) bill and get reimbursed for the new chronic care management code 99490?

Yes, primary care and speciality providers. We are aware of the requirements but want to be sure they will get paid.

If these are hospitalist providers, then they are not providing the ongoing care of the patient. So, they can't bill CCM.
If they are primary care providers that are employed by a hospital, yes.
