Prolonged Service Codes in POS 12

Hello,

One of my clients reported 99354 along with an E/M in a Medicare patient's home. We coded 99349 and 99354 with POS 12 however the 99354 was rejected in the scrubbing process. I can't seem to find any official ruling that excludes POS 12 however, none specifically supporting it.

I'd appreciate any guidance offered, thanks!
