Foreign Object Removal of Vagina

I am having a hard time trying to ascertain whether or not there is a code for removing an object from the vagina, i.e. tampon. I have the diagnosis of 939.2 but am looking for the CPT® code for the actual removal. Can anyone help? Thanks!!

Last I was aware, this falls back to an appropriately documented E&M code

Not separately billable from an evaluation and management code

So I would either code the E/M code or the removal? Could I use Modifier 25 for both?

Only bill the E/M. The physical exam will cover the removal.

Look at 57415-removal of impacted foreign body under anesthesia (other than local)-if anesthesia was not used then use an E/M code

58562 is removal with hysteroscopy
