incident-to billing and non-physician providers who are not NPPs or PAs

Can services provided by a non-physician provider who is not a nurse practitioner or physician assistant be billed using the e/m series as incident to? For example, if a nutritionist is providing nutrition services for a patient diagnosed with celiac disease, and the services otherwise meet incident to criteria (i.e., doctor is available and overseeing the service)?

Or is it the case that E/M cannot be billed by any provider that is not an MD/DO/PA/NP, even if intended as an extension of the physician's services?
