Removal and replacement of neurostimulator leads
Our doctor removed old neurostimulator and replaced with new unit and 2 leads. How would we bill for removal and replacment? Can we bill for each lead removal? If we can bill for each lead removal, do we bill 63661 on two lines?

63661 is reported only if it is removed without replacement.
