Multiple Facet Joint Injections

In the CPT® book for third level facets the decription of codes 64492 and 64495 says "third and additional level(s) (List separately in addition to code for primary procedure) Then is says do not report these codes more than once per day.

We had another source tell us we needed to code for what was actully performed.
For instance if five lumbar levels are done to report 64493, 64494, and then 64495 X 3.
Any input on this would be greatly appreciated.

CPT® codes 64492 and 64495, the add-on codes for the third and any additional levels, may only be reported one time per day.

If you had a 4 level lumbar facet joint injection, your coding would look like this:

64493
64494
64495

The code 64495 is for "third AND ANY additional level(s).... which means you can only bill a max of this code ONCE. You don't code any additional 64495's. You will never code 64495 more than once unless you are performing this on the left side and then on the right side. In that case, you'd report:

64493-LT
64493-RT
64494-LT
64494-RT
64495-LT
64495-RT

But if you are injecting one side, you'd bill it 1 time.

Modifier 50 is also appropriate if you are injecting bilaterally, it just depends on the payers and how they want these injections billed regarding the modifiers.
