quick question about sti screenings

if dx on lab order for GC and chlamydia are both "screening for venereal disease", but you know that the correct screening code for the chlamydia would be v73.88, can you go ahead and code that without a progress note? and say if you look at the progress note to justify that code, and there are symptoms related to the screening, can you go ahead and just code the symptoms, since it wouldn't be a screening (even though the ORDER says screening)?
