Modifier

You charge a nurse visit/25 for IV Infusion Therapy. If you charge 96365 and 96366 is there a modifier on the 96365?

No. The 25 does go on the E&M to designate a significant, separately-identifiable ( and documented) service by the nurse beyond the infusion itself. The 96365 is an "initial" therapeutic infusion, and 96366 is designated as an add-on, so no modifier on either (unless other services are billed, i.e., chemo / hydration.)
