Modifiers with E/M and medication injection

Patient comes in for their office visit and renewal of RX medications. Doctor decides to give injection of testosterone medication. This should be billed as follows, correct? :

99214-25 E/M),
J1080 (testosterone),
96372-59 (intramuscular injection fee)

take the modifier 59 off the 96372. the modifier 25 has already been added to the office visit to allow for the services of the procedure. modifier 59 is used to identify procedures that are not normally reported together, but are appropriate under the circumstances (hence the word procedures!!!, which does not mean office visits)

just another FYI, I would only use the DX code as to why the patient is recieiving the testosterone shot on the 96372 and the J code, but not on the office visit 99214, will help with getting the procedure paid:)
