Please look at 27040, it may work for you.

You need to look at the codes 11400-11406 as each lesion removed is done by measurements in cm. if it was less than 0.2 cm or less it would be 11400 and so forth. If you do not know the measurements ask your physician and it needs to be documented in the procedure notes. :)
