Hospice Billing

Do we need to have the hospice dx in any particular spot if we’re billing GV? For example: TCM follow up where pt was admitted to hospice on return to assisted living - hospital discharge dx was acute respiratory failure. Would the acute problem be listed as primary with the GW modifier? Does his admitting hospice dx of dementia need to be listed too?
