would you code both 414.00 and 414.04 if it is documented as blockage in native artery and the LIMA?

414.00 (Generic code), Coronary atherosclerosis of unspecified type of vessel, native or graft: Assign this code if the documentation does not specify the disease’s location. This code has been a frequent area of errors. Only used if disease location is not specified.
414.01, Coronary atherosclerosis of native coronary artery: Assign this code if the documentation specifies the CAD is of a native coronary artery. Code 414.01 is assigned if the documentation specifies that the CAD is of a native coronary artery or if there is no history of a previous coronary artery bypass graft (CABG).
414.04, Coronary atherosclerosis of artery bypass graft: Assign this code if the documentation specifies CAD of an internal mammary artery or other artery used as a bypass graft. . Code 414.04 is assigned when the CAD involves an artery (mammary, brachial, etc) used as a bypass graft.
