Infusion Authorization

I need some clarification and I hope someone can me with Chemo infusion and entering authorization in an EMR record:

When we give infusions whose name should be entered in the "Authorized By:" drop box? The note is opened under a midlevel provider but the orders are by the physician. Should the midlevel provider that is covering be entered as "Authorized By:"? Or should the ordering physician be entered as "Authorized By:"?

Any input would be greatly appreciated:)

I would always make the 'official' everything under the provider who is ultimately responsible. If you intend for the mid level service provider to be supervised by a physician, then enter the auth under the physician.
