lost colon polyp

My general surgeon removed a colon polyp with hot forceps but the polyp became lost. His operative states that to him that it appeared to be adenomatous. The patient has Medicare and a personal history of colon polyps. Should I just bill V76.51 & V12.72 or add 211.3 as well with 45384?

i would code 211.3 , if the patient documented that is was a polyp code it. if there is any indication that the md is unsure it was a polyp don't code it.
