What is acceptable documentation for inpatient shared service with NPP?

We have been billing our inpatient visits (not consults) under they physician. Our billing department now wants to bill under the PA because they say that the physician isn't doing a "substantive" amount of work and documenting that.

I think substantive means "necessary"/"of substance", but they interpret it as shared - almost half and half.

Does anybody know what the acceptable documentation is for billing shared service under the physician at 100%?

Shared service for Medicare must have a documented face-face by PA and MD, same day--otherwise the combined entries = level of care. Billing department may not fully understand.

The physician does a full face-to-face with the patient and co-formulates the plan.My billing people essentially want 2 notes.

I saw this on one of the hospital medicine websites. My billing people say this isn't enough. So my question is, how much is enough? What is the proper documenation:

“I have personally shared in the visit of Mr/s.____ along with ____PA—C providing face-to-face (or bedside) participation in the evaluation and management. I _____________ (document something you actually did)”

Then document:
? That you “agree with the E/M as documented” and then co-sign the note
Or
? Key elements you wish to elaborate upon
Or
? Additional elements you wish to combine with the PA note to increase the E/M code
