Neurostimulator leads removal and placement of new

I am very new to pain management and need help with the electrode replacement.

procedure was thru previous laminotomy and doc removed two previously implanted electrodes (requiring debridement) and the implantation of two new electrodes.

My doc has 63661 x 2 and 63650 x 2 which I know can't be billed together. Can anyone help or advise?

[would be more appropriate unless performed with laminectomy then]
