Annual Physical Exams

If a patient comes in for their annual physical exam and the provider prescribes meds for an existing condition do we include that dx on the claim or still just bill with V70.0?

Bill the proper Office Visit with a modifier -25 and the diagnosis code for the problem the prescription was written, in addition to the Preventive Medicine code.
