auditing

I am looking for some rules to auditing. I went to a seminar that stated if you have two(history/exam) in one column, but the medical decision making is in another column then you have to pick the lower column to bill the e/m? I am new to auditing and can use any information that you have. Also would like the table/grid that has the columns listed for history/exam and medical decision making and instruction as to how it works if they are all in different columns, how to know what should have been billed.

It takes some time to get a good grip on using an audit table and leveling tool. But once you understand it, it is golden. The key is a good audit table. There is different criteria for new or established pts. With a new pt, the criteria is you must meet 2 out of 3 elements. So wherever you have 2 (from history/exam/MDM) that is your code. If 3 are spread your code is the middle one. For an established pt. your criteria is you must meet all 3 elements, that occurs where your lowest element falls.
