Hello,

An established patient of the Doctor was discharged from the hospital and advised to follow up with their primary, is there a special code than we can bill for?

If the primary has seen within 3 years, established patient billed, no special code.
If discharging physician is primary care doctor, bill follow up visits as an established patient. Otherwise, I do not understand the question.
