Does the Guaiac procedure require an attached diagnosis to prove medical necessity. If so, is there a list that can be used as a reference. I bill for PCPs, so I get many of these procedure when physicals are performed.

If screening, typical diagnosis codes are V76.41 or V76.51

If diagnostic, 578.1 (blood in stool) or anemia might be indications.
