Vital signs

I have a NP who never puts vital signs and any of her acute or progress notes
for our SNF residents.

I told her that they are required in order to have an eligible note for coding purposes. She keeps telling me they are not necessary.

Do I down code her work or just not charge for them or do I do what she says and leave it as she codes it. I am getting feedback from the GM for this.

Any advise would be greatly helpful.
