TCM at global periods

Kindly help me on TCM billing i have doubt as follow;

A patient was admitted to the hospital for a UTI and discharged 1/27/14. The patient was seen in the office on 2/3/14 for her initial hospital follow up visit and Dr Bloom was planning on billing 99496. The patient, today 2/6/2014, fell and was readmitted to the hospital for a broken bone. Since the patient was readmitted to the hospital within the original TCM time frame, are we still able to bill it? And then if the patient is discharged from the hospital in a few days, also bill for TCM 30 days from that discharge too?? Or since the patient was readmitted, does it cancel out the original TCM time frame and start over from the new discharge?

Please advise.

99496 for UTI? Was the patient of HIGH complexity? Underlying medical conditions and highly complex MDM? psychosocial issues?

Is first discharge billable for TCM, because 30days overlap the next second discharge global period
