smoking cessation

When billing 99213 and 99407 with modifier 25, should there also be a modifier on 99407? Medicare keeps denying the 99407. When I called Medicare, I was told I need a modifier on 99407 but of course they would not tell me what it was. Any help with this would be greatly appreciated.

When reporting any significant and separately identifiable evaluation and management service on the same date as tobacco-use cessation counseling, append modifier 25 to the evaluation and management code.
Look at diagnosis codes submitted, look at frequency allowances. Cannot think what modifier they are wanting. Looks like another stall.
