using modifier 25 with ekg

does anyone know which payors require modifier 25 to be billed with an office visit when an ekg is billed?

I don't use it when doing an ekg for any payors.

I agree with rphelps.

Coding rules do not require a modifier on the CPT® code when a diagnostic test is done. Some payers have edits that require them for some diagnostic tests but it's wrong coding!
