charge 20912 times 4?

Medicare has a MUE of "1" for this procedure code. Graft has to be obtained through a separate incision and not considered part of primary procedure/same day. So, the answer is bill only once. If a septorhinoplasty were performed then the septal graft would not be separately reported.
