LCD L32456 PreOp exam for Part A at NGS

Here is the overview of the LCD I have a question on. In what circumstance would a provider bill these services to Medicare Part A?


Noncovered Preoperative Services (L32456) (A51616)
The coverage of services defined as “reasonable and necessary” applies to all diagnostic procedures, with
the exception of Medicare covered preventative and screening services. The existence of policies or
protocols in hospitals or other providers, requiring the routine use of these tests, in and of themselves,
does not justify coverage.
The use of diagnostic testing as part of a preoperative examination, where there is an absence of signs or
symptoms indicating a need for the test, is not covered
