Can you file procedure codes 96365, 96375, and 99284 together when filing to Medicare?

CPT® has a note prior to these codes (96365 and 96375) "These codes are not to be reported by the physician in a facility setting".. The ED is a facility setting, so, the answer is no for physician billing.
