Doctor's note needed by patient

If a patient only comes in to see the provider to get a note stating they are fit to go back to work or back to school, what DX is used? Would it be Z02.79 "encounter for issue of medical certificate? Does a note from the Provider constitute a "medical certificate" or does an actual certificate/form have to be completed?
