Medicare as Secondary Payer

I have a patient that has an HMO as his primary insurance carrier and Medicare as his secondary carrier. My question is this, who is responsible for the patients copay, does the patient pays his own copay or do I bill Medicare and do they allow and pay on copays? Any help with this question would be greatly appreciated.
