11980 Hormone Pellets

Please give us some help on coding this for Medicare payment. We have a client in the state of KY that continually gives these to male patients with the S0189 for the 75 mg dosage. We are having Medicare denials and of course this is left up to the Medicare carrier but we are trying to figure out what we are doing wrong. The physician gives the procedure continually and feels that Medicare should pay appropriately.

Any suggestions would be helpful.
