4th and 5th surgical procedures

Can you please tell us what is the standard form of multiple surgical reimbursements. It's our understanding that the following applies, can you please clarify. Thank you

bilateral 150 percent

1st surgical procedure 100 percent

2nd - 50 percent

3rd - 25 percent

4th and 5th - ????

Any help would be appreciated. We understand that value is based on our contracts. Thanks again

For Medicare it is 100% - 1st, then 50% for 2-5, then by report over 5
