epilation for trichasis

Medicare has always paid for procedure code 67820 for multiple areas we have always billed if done in more then one area 67820E4 on one line and 67820E251 on a second eye and it was paid. How Medicare is denying both lines. When I called I was told they will only pay for one epilation per day. When asked why didn't they pay for even the one line she couldn't answer. I was told by other offices they are billing them with RT /LT on two lines and they are getting paid. Is this correct and do you still append a 51 modifier ?
