Cervical Spine

Below I am going to list the procedures that the doctor dictates on the operative report... I really need some help to make sure I am coding this correctly. thank ya'll in advance....

1. Anterior cervical diskectomy and fusion at C4-C5
2. Anterior spinal nonsegmental instrumentation from C4 to C5 with 18 mm small stature Synthes locking plate.
3. Tricortical allograft placement at C4-C5.
4. Intraoperative fluoroscopy with interpretation.
5. Drain placement.
6. Radical diskectomy, C4-C5
