Snf

Pt requested PCP to visit her at the SNF. Can I bill an E/M with POS 31 with no problem? I need some feedback. Thanks!

If the Facility provider has not seen the pt in the SNF that day, and the PCP has contacted the Facility provider and received permission and gained facility privileges, (they need to submit credentials (copies of license, malpractice ins., and DEA license, to that facility), then the PCP can see the pt at the SNF. This would be coded as a consultation if the pt is being seen by another provider while in the SNF. They must do a consultation note and have it added to the chart.
My question would be, why isn't the PCP following the pt in the SNF? Medicare justification for the SNF stay is that the pt is receiving Skilled Services by therapy. The facility provider must certify the need for rehab therapy upon admission, at 14 days and then every 30 days for the next 4 months. While on Skilled Services, the pt must be seen on Admission for an H&P then every 30 days for 4 months. The need for SNF services justifies the need for additional provider care, so in my facility the provider sees the pt every week for the first 30 days (4 times in the first month) on an as needed basis. Some only need to be seen once, some more then 4.
